Life Transition Services, LLC started in 2002 to be one place that Seniors, Caregivers, and their families can get answers about transitioning to long term care. Since then, we have been serving families needing long term care in Pennsylvania and beyond to solve their complicated issues.
We offer a personal guide not just the map. Rather than providing a “to do” list or piecemeal solutions, Life Transition Services, LLC offers an experienced long term care professional who helps families implement a custom plan that meets their needs.

Jeffrey M. Jones, CSA, PE is the CEO of Life Transition Services, LLC. He is in charge of planning and administration for families that will be applying for government benefits to pay for long term care. This includes: documentation and organization, estimating spend-down and target date of benefits, coordinating between family and caseworker, discharge and admissions offices, and social workers. He has developed a methodology to transition a family’s retirement to the requirements of long term care.
Mr. Jones has a background in information systems design and implementation plus engineering for the past 25 years. He has developed systems for government and industry including research, engineering, and pharmaceutical industries. He has a Masters Degree in Electrical Engineering from Drexel University and is a registered Professional Engineer. He has been a Certified Senior Advisor since 2002.
