● Conduct initial assessment based on past and present medical history, cognitive and mental health status, legal requirements, daily living activities assessment, home safety assessment, and general financial assessment.
● Develop a comprehensive care plan based on initial assessment results to determine the patient's needs, identify concerns and provide recommendations.
● Provide home visits (no need for caregivers to go to an office)
● Provide more personalized plans and not set limitations like insurance companies.
● Provide ongoing consultations based on the patient’s current needs at an hourly rate.
● Act as a liaison for families requiring assistance in seeking services.
● Act as a public health educator regarding the healthcare system.
Based on my personal experience caring for my mother with Alzheimer’s, diabetes and heart disease, I understand that the demands of care-giving can become overwhelming. Navigating the healthcare system and insurance companies are very complicated and confusing. I am also aware that choosing care for a loved one is emotional and not a one-size fits all. Having vast experience as a pharmacist and a public health analyst, I will be able to assist clients through difficult decision making processes.
